What is the Universal Health Care Nursing Home Diversion Program?
Our Nursing Home Diversion Program is a Social Service program with the objective
of enabling our enrollees, to remain at home or stay in assisted living facilities
rather than in nursing homes. We provide a comprehensive range of services to our
participants in their own familiar environment consequently enhancing their Quality
of Life.

How does it work?
Our Diversion Program provides services to our elderly through their Medicare and
Medicaid benefits. There is NO premium to receive these benefits.
If an enrollee lives in an assisted living facility (ALF), our program will pay
a share of the cost to the facility. The participants will be able to keep their
Medicare doctor and hospital.
Once qualified, participants are assigned a Case Manager to assist in accessing
available community resources.
How can I participate in the program?
To qualify, an elderly must meet the following:
- * Must be 65 years or older
- * The medical requirements determined by the Department of Elder Affairs (DOEA).
- * The financial requirements determined by the Department of Children and Families
(DCF) i.e. eligibility for Medicaid and Medicare Part A & B.

How is Universal Health Care Nursing Home Diversion Program different from other
diversion programs?
A dynamic and responsive personalized Plan of Care is developed for each of our
elderly participants to assist them in their ever-changing needs.
Low case load for our compassionate and knowledgeable Case Managers affords them
more quality time to spend with our participants and their families/ responsible
parties especially during difficult situations.
Our Case Managers are available via cell phone during working hours. After office
hours, a Case Manager can be contacted through an emergency cell phone.
Consumer Satisfaction Surveys are conducted on a regular basis to ascertain that
our Diversion program continuously and consistently meets our participants needs
and expectations.
