Each year, nearly one third of adults over 65 experience a fall. Falls are the reason
for 40% of admissions to nursing homes. Universal Health Care has a new Fall Prevention
Program. The goal of the Fall Prevention Program is to educate members who are at
risk for falls. We want to encourage awareness of falls and fall risks. We will
also provide information about specific chronic medical conditions and fall prevention
“safety tips.”

This program is available for members with:
|
|
• A history of falling
• A fall related injury
• An age of 80 years of age or older
• A fall risk based on the Health Risk Assessment (HRA)
• A Universal Health Care Medicare membership
|

Universal Health Care wants to help keep members safe in their homes and maintain
their independence. We do this by identifying safety hazards in the member’s home
environment and daily routines.

The Fall Prevention Program includes:
|

• Calls from a case manager to determine the risks for the falls.
• Education about fall prevention
• A home health safety evaluation if appropriate
• Health and safety educational materials
