Vitalifesm Program Information
Your coverage through the Medicare Masterpiece Plan includes participation in Universal
Health Care's Vitalifesm program. This program provides an opportunity
to join one of our Fitness Centers at no additional cost to you. Members may also
enjoy access to seminars with Registered Dieticians and full access to a web - based
nutritional program. The entire Vitalifesm program is provided at no additional
cost to you.

UHC's Vitalifesm program provides free membership to
fitness centers in your area.

To take advantage of this service, simply fill out the 'Fitness Center Registration'
form below and have your Primary Care Physician sign it. Once completed, mail it to the
address provided at the bottom of the form. After you receive your membership card
in the mail, you may take full advantage of the fitness center programs, classes
and one free personal training visit. Please, always swipe your Wellquest ID card
when entering your gym for tracking purposes. This helps us keep the free gym membership
available to you by confirming that our members are using the Vitalife benefit.
You are not required to join one of our fitness facilities if you do not wish to.

to our 'Vitalifesm partner'. You may use this program
to learn about healthy habits, track weight loss or gain, set goals, and follow
your progress.
